The Seniors’ Community Outreach to Vulnerable Seniors Funding Initiative supports community-based projects located within the Toronto Central LHIN Boundaries that target outreach services to frail senior’s population groups.
This project focuses on health promotion, chronic disease management, service coordination etc. Its objectives are engage target seniors in the community, volunteers’ involvement, health services partnership building and referrals, process of goals setting for seniors.
2012 received the funding as trail project. Based on its success, FIG Link gets funding approval each year till now. The name of the project changed a little bit in 2018.
